I, the undersigned participant (and my parent or guardian if I am under the age of 18), acknowledge and understand that boxing is a high-risk sport and I am voluntarily participating in these activities with knowledge of the danger involved. I hereby agree to accept any and all inherent risks of property damage, personal injury, or death.

I understand that in the event of an accident or injury, Martinez Boxing Houston and its employees, representatives, agents and affiliates are not responsible. I hereby release Martinez Boxing Houston from all liability for injuries sustained or illnesses contracted while attending or participating in any activity. I agree to indemnify Martinez Boxing Houston for any loss or damage, including any that results from claims or lawsuits for personal injury or death.

I have read and understood this waiver and release from liability. I voluntarily agree to the terms and conditions stated above.
